Pharmacodynamics: Spectinomycin is an aminoglycoside antibiotic that exerts moderate inhibitory effects against various Gram-negative bacilli, such as Escherichia coli, Salmonella, Shigella, and Proteus. It is effective against Streptococcus, Streptococcus pneumoniae, Staphylococcus epidermidis, and certain Mycoplasma species (e.g., Mycoplasma gallisepticum, Mycoplasma meleagridis, Mycoplasma synoviae, Mycoplasma hyorhinis, Mycoplasma hyosynoviae). It is largely ineffective against Streptococcus viridans and Staphylococcus aureus. Pseudomonas aeruginosa and Treponema are generally resistant. Widespread resistance to spectinomycin exists among enteric bacteria, but no cross-resistance with streptomycin has been observed.
Pharmacokinetics: Only about 7% of the orally administered dose is absorbed, yet high concentrations are maintained in the gastrointestinal tract. Subcutaneous or intramuscular injection results in good absorption, with peak blood concentrations reached approximately 1 hour post-administration. Tissue concentrations are lower than serum concentrations. The drug poorly penetrates the cerebrospinal fluid or ocular tissues, exhibits low plasma protein binding, and is primarily excreted unchanged via glomerular filtration.
